What are my chances of getting into a top 20/30 PhD program with perfect grades and experience as teaching and research assistant, but no publications/scholarships? Did my undergrad at a top 50.

Most students admitted to top 20 programs do not have publications. Lots have senior theses that they use as writing samples.

It's not necessary but some students do have publications. This doesn't apply to the OP, but if you have weak grades or letters from people who aren't known, a publication may be necessary.
