This is on the search committee, not the applicant. No need to be so pointed toward the applicant.
The person failed to submit letters of recommendation. Note it is not a letter, but letters.
The search committee and the Dept moved forward the person all the way without her letters. Note her references failed to write a letter on her half, and the Dept did not follow the Guidelines and Procedures for Academic Appointments (https://diversity.illinois.edu/NewSearchManual/search_guidelines_and_procedures.pdf).
